Information We Collect
Depending on your relationship with us, we may collect personal information such as your name, contact details, date of birth and emergency contact details. We may also collect sensitive information relevant to providing support, including information about your disability, health, support needs, NDIS funding, goals and preferences.
We may maintain case notes and records relating to the supports we provide. Information may be provided directly by you or, where appropriate, by family members, nominees, support coordinators, plan managers or other people involved in your supports.
We only collect information that is reasonably necessary to provide our services, communicate with you or meet our legal and administrative responsibilities.
How We Use and Share Information
We may use your information to understand your needs, preferences and goals; provide and coordinate support; communicate with you and people involved in your supports; maintain appropriate records and case notes; manage timesheets, invoicing and administration; respond to enquiries, feedback or complaints; and maintain the safety and quality of our services.
Where appropriate, information may be shared with people involved in your supports with your consent, or where otherwise authorised or required by law. We aim to share only the information reasonably necessary for the relevant purpose.
Storage and Security
Little Ladder Supports maintains electronic records using secure digital storage, including Google Drive, and may also maintain physical participant files.
We take reasonable steps to protect personal information from loss, misuse, unauthorised access, modification or disclosure. Access to participant information is limited to people who require it for their role, and physical records are kept securely.
Access, Corrections and Privacy Concerns
You may ask to access personal information we hold about you or request a correction if you believe information is inaccurate, incomplete or out of date.
